2. Choose Your Preferred Method to Complete the Forms

• Option 1: Fill Out Forms Digitally
  Open the PDF on your computer or mobile device, and fill in your details directly in the form fields. Once completed, save the file to your device.
• Option 2: Print and Complete by Hand
  If you prefer to fill out the forms by hand, print the PDF, and complete each section with your information.
• Submit the Completed Forms
  • By Email: Once your forms are completed:
    • If filled out digitally, save the completed file and attach it to an email.
    • If completed by hand, take clear photos or scan each page.
    • Send the digital forms or photos/scans as attachments to bellevueeyecare@comcast.net.
  • In Person: If it’s more convenient for you, feel free to bring the completed forms with you to your appointment. Having them ready will streamline your check-in process, allowing us to focus more time on your eye care needs.
